Risk Assessment Form

This is the form to assess the risk of all projects to be performed in the Universiti Malaya High Voltage Lab (UMHVL). Users who wish to use the UMHVL must obtain permission from Professor Ir. Dr. Hazlee Azil Illias (email: h.illias@um.edu.my) and fill in the risk assessment form. Projects with high risk and no additional safety measures are not allowed to be conducted in the UMHVL. Please use your Microsoft 365 institutional account to fill in the form.

Equipment Booking Form

Please use this form to book the equipment in the Universiti Malaya High Voltage Lab (UMHVL). Users who wish to use the equipment in UMHVL must obtain permission from Professor Ir. Dr. Hazlee Azil Illias (email: h.illias@um.edu.my). Please use your Microsoft 365 institutional account to fill in the form.

Asset Movement Form

This is the form to move any asset out of Universiti Malaya High Voltage Lab (UMHVL). The move can be due to borrowing equipment, servicing/repairing/calibrating equipment or transferring equipment to other location. Users who wish to use move any asset out of UMHVL must first obtain permission from Professor Ir. Dr. Hazlee Azil Illias (email: h.illias@um.edu.my).
